For those who are members of the CSOs/NGOs

1. Endorsement Letter from the Organization
2. Brgy. Clearance
3. Photocopy of Government Issued ID
4. 2pcs 2×2 ID Picture

For those who are NOT members of the CSOs/NGOs

1. Essay explaining how CSO Academy help their future plan in Civic Engagement (Atleast 500 words)
2. Brgy. Clearance
3. Photocopy of Government Issued ID
4. 2pcs 2×2 ID Picture
